To review the current practices and pathways within our community based mental health services, with a view to developing new service models that:

• provide improved access and high quality interventions
• are more inclusive
• promote informed choice
• enable service users to better navigate their own care pathway
• provide value for money

We will evaluate the success of this project by the degree to whcih the new service models are:
Fair
Effective
Personal
Safe

The project will have three distinct stages as follows:

• Where are we now?
  Identifying the current position its strengths, weaknesses, gaps and opportunities as at our "Census Day" of 1st April 2008
• Where do we want to be?
  Articulating the vision by the 15th May 2008
• How will we get there?
  Complete the implementation plan – with tangible tasks linked to timescales and responsible persons – to be completed by 30th June for discussion and review throughout July for the actual implementation from August
